Millennium Physician Group has more than 28 primary care practices, three Imaging Centers and five Lab Draw Stations. We also offer Wellness Programs including smoking cessation, weight management, diabetes education and physical therapy. Millennium Physician Group is one of the most comprehensive primary care groups in Southwest Florida.

We have more than 500 employees on staff throughout our many locations.

Where are your offices located?
We have offices in Englewood, North Port, Port Charlotte, Punta Gorda, Fort Myers and Naples.
