Millennium Expands in Jacksonville

Millennium Physician Group is proud to announce that we have a new primary care office in Jacksonville!  We welcome Mohammad Farooq Taj, M.D., and Rebecca Munzon, FNP, to our team.  Their knowledge of health care, and years in the community, is of great value to our organization.  For more information about our new locations and providers, see below!

Millennium’s Newest Team Members

Mohammad Farooq Taj.,M.D.,  internal medicine physician,  completed his medical degree from the University of the Punjab in Pakistan.  He then completed his residency at Coney Island Hospital.  Dr. Taj speaks Urdu and Punjabi. He has been seeing patients for over 18 years!

Rebecca Munzon, FNP, earned her Bachelor of Science in Nursing from Chamberlain University in Addison, Illinois. She welcomes you to her location.
