WellCare Medicare Advantage Update

Millennium Physician Group will be ending its participation in WellCare Medicare Advantage HMO and PPO Plans by issuing a notice of termination to WellCare of Florida, Inc. and WellCare Health Insurance of Arizona, Inc. (“WellCare”) for the WellCare Medicare Advantage HMO and PPO products, which also includes termination of participation in all WellCare Medicare Advantage HMO and PPO plans originating outside the state of Florida.

After April 30,2023, Millennium physicians will no longer be participating providers in the WellCare Medicare Advantage HMO and PPO networks. Please note this also includes all WellCare Medicare Advantage HMO and PPO plans originating outside the state of Florida.
