NonDiscrimination and Accessibility Statement
Language Services
Part of Millennium’s mission is to connect our patients to a healthier life. This means making you feel comfortable during your visits and making sure you fully understand your provider and your healthcare needs.
Millennium takes reasonable steps to ensure that patients with disabilities to communicate effectively have meaningful access and equal opportunity to participate in our services and programs. We offer qualified interpretation and translation services to our patients who wish to communicate in a different language, have limited English proficiency, or those who are deaf or hard of hearing at no cost. If you need these services or other types of assistance, please let one of our team members know.
Interpretation and Translation Services
All interpreters, translators and other aids needed shall be provided without cost to the patient being served; the patients and their authorized representatives will be informed of such assistance free of charge. Policies and important documents involving medical conditions and treatments can be translated to a patient’s preferred language. This includes financial and insurance benefit forms, waivers and so much more!
Deaf and Hard of Hearing Services
In all situations, patients with hearing loss will be advised that Millennium Physician Group offers, free of charge to the patient, the services of a qualified American Sign Language (ASL) Interpreter. By law, minors are not permitted to act as interpreters during an office visit.
If you believe that Millennium has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity), you can file a grievance with:
