Payment Options

  1. Pay at the office with a credit card, cash, or check.
  2. Set a credit card on file to be charged automatically once an Explanation of benefits (EOB) is received from insurance. Speak to your provider’s office for more information.
  3. Mail check and statement stub to Millennium Physician Group P.O. Box 14000 Belfast, ME, 04915
  4. Make a one-time payment with a credit card by using the statement code provided on the patient’s statement
  5. Scan the QR code on the statement to pay with a credit card
  6. Log in and pay through Athena portal – Select your state of service
  7. Call the CBO (877-856-3774) or reach out to your provider’s office to set up a payment plan.
  8. Self-pay patients are expected to pay at time of service rendered.
