Payment Options
- Pay at the office with a credit card, cash, or check.
- Set a credit card on file to be charged automatically once an Explanation of benefits (EOB) is received from insurance. Speak to your provider’s office for more information.

- Make a one-time payment with a credit card by using the statement code provided on the patient’s statement
- Scan the QR code on the statement to pay with a credit card
- Log in and pay through Athena portal – Select your state of service
- Call the CBO (877-856-3774) or reach out to your provider’s office to set up a payment plan.
- Self-pay patients are expected to pay at time of service rendered.
